Our
programs and community resourcesIn addition to comprehensive health
coverage, as a member of Texas Children’s Health Plan, you will
have access to additional resources and programs. These
resources include our Community Outreach team, Member Services,
the STAR Babies program and our Disease and Case Management
services.
Community Outreach
Texas Children’s Health Plan understands children and the
communities we serve. Our Community Outreach team works with
community-based organizations, such as schools, community
centers and churches, to help fulfill the needs of their
communities by participating in local health fairs and events.
Every year, Community Outreach offers three health fairs to
Houston residents. The locations rotate, but the mission is the
same—give our communities valuable tools to help them stay
healthy. The Outreach team also puts together an annual
back-to-school event which offers families a variety of free
screenings, and most importantly, free school supplies.

Member Services
Texas Children’s Health Plan has an outstanding group of
individuals that work with our members on a daily basis. From
answering basic questions about health coverage to guiding them
through the various programs, our Member Services team is there
to help our members get easy access to the best health care.
Member Services also works to keep our members informed about
how raise healthy children. They provide our members with a
quarterly newsletter, Kids First, to keep members informed on
health topics important to them. Member Services provides our
members with a free 24-hour-a-day nurse help line. Families can
call our nurse help line and speak to a nurse about health
concerns before making an unnecessary trip to the emergency
room.

STAR Babies program
Texas Children’s Health Plan isn’t just for kids! We cover
pregnant women as well. As part of that coverage, Texas
Children’s Health Plan has developed the STAR Babies program.
This program was designed to help soon-to-be moms prepare for
their new baby. The STAR Babies program provides moms with a
monthly newsletter outlining what to expect during their
pregnancy and how to have a healthy pregnancy. Furthermore,
Health Educators are available to help moms-to-be through their
pregnancy by answering any questions they may have. The Health
Educators hold baby showers for the mothers and provide
valuable information as well as free gifts.

Disease and Case Management
Texas Children’s Health Plan offers disease management programs
to members that suffer from diabetes or asthma. These programs were
designed to educate and assist members with their treatment.
Members with asthma or diabetes are sent a quarterly newsletter
along with other educational pieces on how to manage their
condition.
Case Management will help any
member that has special needs because of an on-going illness or
other circumstances. Similar to Disease Management, Case
Management assists members by providing educational
pieces, locating services, coordinating care and helping with
any other requests from the member.
Texas Children’s Health Plan works
to meet every member’s specific needs. As part of our plan,
members receive the best care possible; through educational
pieces, community outreach events and quality customer care.
When choosing a health plan to
cover your family, take peace of mind in knowing that Texas
Children’s Health Plan is the best decision a family can make.
